In order to expedite requests for resale estimates and to provide
information which is as accurate as possible, The Paper Bag Princess
asks that you PRINT OUT AND COMPLETE A COPY OF THIS REQUEST FORM.

Your request must include a photograph. Please insure that each photo
shows a single item photographed on a neutral background. WHEN
APPLICABLE A CLOSE-UP PHOTO OF THE LABEL OF THE ITEM IS ALSO REQUIRED.

Please ensure that your name, address and telephone number appear on
the back of each photo, along with the size and when possible the
original resale price of the item, date of purchase or collection, as
well as the price you wish to receive for the item.

A self addressed stamped envelope must be included should you wish to
have the photos returned.

In order to help you, I require certain basic information relating to
the item you wish to sell, therefore I would ask you to please try to
describe your property carefully.

* Is it a single item, part of a set or from a collection?

* What is the name of the designer?

* Is there a label in the item from the shop that originally carried the item?

* What is the age of the item? The year and season it was designed?

* What is the provenance?

* Please describe the source of your item, especially with regard to
* its original ownership and/or recent history. Did you inherit this
* item from your family, or was it purchased by you in a vintage
* venue?

* What is the fabric? For instance, is your item silk, silk jersey,
* rayon jersey, wool, and cashmere?

* What size is it?

* Are there any markings or signatures on the item? If so, please
* describe, sketch or send photographs of any signature.

* What condition is the item in?  Please detail accurately any damage
* or wear. Although repairs are often difficult to detect, try to
* describe any flaw or other evidence of restoration. For example:
* rips, holes or stains.

* Have you attempted to sell your item through any other venue? If so
* please explain.

* Has your item ever been exhibited or appeared in any publications?

* Please also include any other comments or information that you
* believe might helpful.

* Please take into consideration, the higher the quality of the
* photographs you provide, and the more detailed the descriptions of
* your item, the more accurate my appraisal will be.



Note my appraisal will of course, be subject to change based upon firsthand inspection of your item.
